Hospice care in a private home means the patient stays wherever they live (house, apartment, assisted living) while a hospice agency “brings” medical, emotional, and spiritual support to them and their family.

What our hospice home coordinates

  • Regular visits from a nurse for pain and symptom management, with a hospice doctor overseeing the plan of care.
  • Access to 24/7 on‑call support by phone and, when needed, a nurse visit for urgent problems (severe pain, breathing trouble, agitation).
  • Delivery and setup of medical equipment such as a hospital bed, bedside commode, wheelchair, oxygen, and other needed durable medical equipment.
  • Medical supplies related to the illness (wound care supplies, incontinence products, gloves, etc.).
  • Medications related to the terminal diagnosis and symptom control (for pain, shortness of breath, anxiety, nausea, etc.), usually provided and delivered through the hospice benefit.
  • Hospice aide/homemaker visits to help with bathing, grooming, and some light personal‑care related tasks.
  • Social worker support for emotional needs, advance care planning, and connecting with community resources.
  • Chaplain/spiritual care if the patient or family wants it, plus bereavement support for family after death. Our caregivers provide one-on-one, hands-on care (helping with meals, toileting, repositioning). We are basically an extension of a facility but with a dedicated, more personalized approach for your loved one.
