Keeping Your Gums Healthy: Hydrogen Peroxide vs. Carbamide Peroxide

When people think about keeping their teeth healthy, brushing and flossing usually come to mind. But for patients with gum disease, there are additional tools that can help improve oral health. Two of the most common ingredients used in professional dental care are hydrogen peroxide and carbamide peroxide.

Both hydrogen peroxide (HP) and carbamide peroxide (CP) gels are effective antimicrobial agents that help reduce plaque and alter the microbial environment in periodontal pockets, the spaces that form between the teeth and gums as gum disease progresses.

Efficacy Comparison

HP acts rapidly to release oxygen, helping reduce harmful bacteria, but breaks down quickly. CP is more stable, breaking down into HP and urea, which buffers the pH and provides a slower, longer-lasting release of active oxygen.

  • Antimicrobial Action: Both peroxides disrupt bacterial cell walls and penetrate disease-causing biofilm. HP offers an immediate “shock” to anaerobic bacteria, while CP provides sustained-release oxygen over several hours.
  • Tissue Penetration & Healing: HP gels (typically delivered via custom prescription trays have clinical studies showing that a 1.7% HP gel, combined with scaling and root planning (SRP), significantly reduces pocket depths compared to traditional SRP alone.
  • Safety & Irritation: Because CP releases HP and urea, it is often gentler on gum tissue at equivalent active-oxygen levels. HP, however, can denature protein matrices more aggressively.

Formulation Conversion

It’s important to remember that these products work differently. HP acts immediately, while CP must first break down, so the concentrations are different. For example, a 10% CP gel produces a similar amount of active hydrogen peroxide as a 3% HP gel.

Delivery Methods

Just as important as the product itself is how it is delivered. Studies have found that peroxide gels are most effective when placed directly beneath the gumline (subgingival) pockets using sealed, custom-fitted dental trays. This approach is more effective than simply brushing or rinsing.
