As a chiropractor, I hear this daily. And honestly? It’s a completely understandable way to describe how you feel. But as an “expert contributor,” I have a little secret to share: your bones aren’t actually sliding out of place like a loose Jenga block.
If we picture our spines like a Jenga tower, we imagine a vertebra gets tapped slightly “out.” You visit your chiropractor, we tap it back “in,” and all is well! But if that were true, why does it seem to go “out” again and again?
While true alignment issues certainly exist—such as scoliosis, a misaligned knee joint, or a structurally shifted vertebra—your spine isn’t a stack of loose wooden blocks. It is a highly dynamic, living system.
So, what is actually happening? Let’s talk about what “out” really means, and why understanding the reality can completely change how you approach your health.
The New “Out”: Movement Dysfunction
If we want to get scientifically accurate, the best way to describe what chiropractors actually treat is a movement dysfunction. It’s a simple concept, but it is at the very core of musculoskeletal health.
- Flexion vs. Extension: A joint might bend forward beautifully, but refuse to arch backward. That is an extension dysfunction.
- Rotation: Your neck might turn to the right just fine, but pinch and seize when you try to look over your left shoulder. That is a rotation dysfunction.

Another flaw of the “I’m out” model is that it places all the blame on the bone. Patients often ask me, “Doc, I don’t know if I should be here—is this a bone issue or a muscle issue?”
The truth is, it’s always both.
A joint is a team. For your spine and extremities to move smoothly and pain-free, an entire orchestra of tissues must play in perfect harmony:
If one player is out of tune, the whole symphony suffers. Whether your issue started in the muscle or the joint, the resulting movement dysfunction involves the entire system.

- Biomechanically: It immediately helps restore restricted range of motion to locked joints.
- Reflexively: It stimulates specific nerve endings (mechanoreceptors) in the muscles, signaling them to relax reflexively.
- Physiologically: It triggers a local cascade of chemical changes that help decrease inflammation and boost blood flow to accelerate healing.
- Neurologically: It wakes up the connection between your central nervous system (your brain) and the affected area, improving your “neuroplasticity”—which is just a fancy term for your brain’s ability to relearn how to move correctly.
