When an older adult returns home after a hospital stay, the first few weeks are often critical to recovery. Many hospital readmissions can be prevented with proper planning, support, and attention to daily routines. Family members can play an important role in helping loved ones stay healthy and avoid an unexpected return to the hospital.

1. Stay on Top of Medications

A change in medications is one of the most common reasons older adults experience complications after a hospital stay. Families should carefully review discharge instructions, understand what each medication is for, and use pill organizers or reminder systems to help ensure medications are taken correctly.

2. Keep Follow-Up Appointments

Follow-up visits allow doctors to monitor recovery, address concerns early, and adjust treatment plans if needed. Missing these appointments can allow small problems to become serious health issues. Scheduling transportation in advance can help ensure appointments are not missed.

3. Create a Safe Home Environment

Falls and injuries can quickly lead to another hospital visit. Removing tripping hazards, improving lighting, installing grab bars, and ensuring frequently used items are easily accessible can make the home safer during recovery.

4. Understand the Role of Home Care and Home Health Care

Many families assume home care and home health care are the same thing, but they serve different purposes. Home health care is typically prescribed by a physician and provides skilled medical services such as nursing care, physical therapy, or wound care. Home care focuses on non-medical support, including meal preparation, personal care, mobility assistance, medication reminders, and companionship. Together, these services can create a strong support system that helps older adults follow care plans, maintain independence, and reduce the likelihood of returning to the hospital.
