Help them keep their independence and stay safe.

Small changes in a parent’s or loved one’s behavior can mean their home is no longer as safe as it once was. Dementia gradually affects judgment, memory, and spatial awareness, so families often adapt without noticing rising risks—until a crisis hits. These precautions will help keep them safe and confident.

Here are 12 clear signs it’s time to act:

1. Repeated stories or questions

Telling the same story three times in one visit often signals short-term memory gaps that now affect daily safety tasks like taking medication or locking doors.

2. Stove left on

A forgotten burner is one of the most common—and serious—fire risks. Knob covers or automatic shut-off devices can reduce the danger.

3. Wandering or getting lost outside

Being found blocks away (sometimes in pajamas and headed to an old workplace) is not a fluke. About 6 in 10 people with dementia will wander at least once.

4. Unexplained bruises, limps, or near-falls

Impaired judgment raises fall risk. One fall roughly doubles the chance of another.

5. Everyday objects become trip hazards

Rugs, cords, clutter, and dim lighting turn into real dangers when balance and attention decline.

6. Medication mistakes

Missed, doubled, or misplaced doses are common and dangerous. A day-and-time pill organizer plus daily check-ins help prevent problems.

7. Bathroom feels risky

Wet floors, tub steps, and missing grab bars make this the highest-risk room. Add grab bars, non-slip mats, and a shower chair.

8. Nights get harder (sundowning)

More confusion, restlessness, or door-pacing after dark means better lighting, a bed alarm, or a calmer evening routine is needed.

9. Keys or coats near the door trigger leaving

These items can prompt someone to head out without a clear destination. Move them out of sight and add door sensors or chimes.

10. Unsafe use of tools or appliances

Cutting the wrong way or leaving an iron on overnight means those items need safeguards or simpler routines.
