Hormone Optimization 101: What It Is, Who It’s for, and What to Expect

“Is it my hormones?” It’s one of the most common questions I hear in my practice—often from patients who’ve been feeling off for months, sometimes years, without a clear explanation. Fatigue that won’t lift. Weight that won’t budge despite real effort. Mood swings, sleep trouble, low motivation. The answer, more often than people expect, is: quite possibly, yes.

As an Internal Medicine Physician, I’ve spent nearly a decade caring for patients across a wide range of medical needs, and since 2017 I’ve focused much of my work on hormone optimization as part of a broader approach to wellness. I trained at Chicago Medical School and completed my residency at Northwestern Hospital in Chicago—a foundation in evidence-based medicine that still guides how I evaluate every patient who comes through my door.

Hormones are chemical messengers that influence nearly every system in the body—metabolism, energy, mood, sleep, libido, and more. When they fall out of balance, whether due to age, stress, thyroid function, or other factors, the effects can ripple through nearly every part of daily life. And because symptoms of hormonal imbalance often overlap with so many other conditions, including thyroid disorders, depression, and even simple sleep deprivation, it’s an area that’s frequently overlooked or misdiagnosed. Patients often spend years bouncing between specialists before anyone connects the dots.

Hormone optimization isn’t about chasing a number on a lab report. It’s about understanding your full clinical picture—symptoms, history, lifestyle, and lab values together—and developing a plan that’s actually built for you. For some patients, that might mean addressing thyroid function. For others, it’s about optimizing levels that naturally decline with age, such as estrogen, testosterone, or progesterone. Still others may benefit from a combination approach that also considers adrenal health and metabolic factors. The goal is always the same: helping you look, feel, and function at your best, safely and sustainably.

I want to be clear about something: hormone therapy isn’t right for everyone, and it isn’t a shortcut. It works best as part of an individualized plan that also considers nutrition, activity, sleep, and stress—the foundational pillars of health that no hormone panel can replace. My approach blends advanced therapies with that broader, whole-person lens, because lasting results come from balance, not extremes. I also believe strongly in ongoing monitoring; hormone levels shift over time, and a plan that works well today may need adjustment six months or a year from now.

This is also an area where I think patients benefit from asking more questions, not fewer. What labs are being used to guide treatment? How will progress be measured? What are the realistic risks and benefits for someone with your specific health history? A good hormone optimization plan should be able to answer all of these clearly, and should always prioritize your overall health and safety over speed of results.

If you’ve been wondering whether your hormones might be playing a role in how you’ve been feeling, the first step is simply having that conversation with a provider who will listen closely and look at the full picture, not just a single lab value. Feeling better isn’t always about doing more. Sometimes it’s about finally understanding what your body has been trying to tell you all along.
