Many people wearing an upper denture would love to replace it with teeth that stay firmly in place—but were told years ago that they simply didn’t have enough bone for dental implants.
For many, that assessment may have been completely reasonable at the time. However, implant dentistry has changed dramatically over the years.
When the upper teeth—particularly the premolars and molars—have been missing for a long time, the jawbone gradually shrinks. At the same time, the maxillary sinus can expand into the area where the tooth roots once were. Eventually, there may be very little bone remaining in which to place conventional dental implants.
Historically, this created a significant problem. Patients often required extensive bone grafting or sinus grafting before implants could even be considered. For many people, the cost, healing time, and complexity of these procedures were simply too daunting.
Today, we have another tool: remote anchorage implants.
Rather than relying exclusively on the bone directly beneath the missing teeth, certain advanced implant techniques allow us to engage stronger bone located farther back in the upper jaw. One example is the pterygoid implant, a longer implant positioned into dense bone behind the upper jaw.
For appropriately selected patients, pterygoid implants can provide additional support for a fixed set of replacement teeth—even when very little bone remains beneath the sinus. In some cases, this approach may also reduce or eliminate the need for extensive grafting.
Why does this matter?
Because some people have spent years—or even decades—believing that a removable denture was their only option.
Pterygoid and other remote anchorage implants aren’t appropriate for everyone. They require careful 3D imaging, detailed planning, and specific surgical experience. But the bigger message is simple:
If you were told years ago that you don’t have enough bone for dental implants, it may be worth asking again. You just might be able to retire the denture cup—and keep your teeth where they belong: in your mouth, not on the bathroom counter.
